History of Present Illness:
A CBC shows WBC count 6820/microliter with differential count of 63 segs, 4 bands, 20 lymphs, 11 monos, and 2 eos, Hgb 13.6 g/dL, Hct 40.8%, MCV 92 fL, and platelet count 212,000/microliter. Serum chemistries show sodium 141 mmol/L, potassium 4.6 mmol/L, chloride 103 mmol/L, CO2 26 mmol/L, creatinine 1.1 mg/dL, and glucose 124 mg/dL. His total CK is 201 U/L with CK-MB fraction of <1%. His troponin I level is 6 ng/mL. A chest radiograph reveals an area of increased density at the right lung base.
Further past history reveals that his father died of a myocardial infarction at age 49. His mother is alive, but suffered a "stroke" at age 52.
