Physical Examination (Robert): He appears in no acute distress. Vital signs: temperature 37.1 C, pulse 80/min, respiratory rate 16/min, and blood pressure 140/85 mm Hg. Lungs clear to auscultation. Heart rate regular with no murmurs. Abdomen non-tender with bowel sounds present. Pain with deep palpation in the suprapubic region. No abnormal findings on rectal exam, with stool negative for occult blood. Pulses equal and 2+ in all extremities, except 1+ for dorsalis pedis and posterior tibial pulses. Focal 1 cm erythematous area on left great toe with loss of overlying skin and eschar formation. PERRLA. EOMs normal. Cranial nerves II - XII intact. No focal neurologic deficits, except for decreased sensation to light touch over lower extremities. Ht 6' 1" (185 cm); weight 262 lb (119 kg); waist circumference 42 inches (107 cm), hip circumference 39 inches (99 cm).

Heart rate regular with no murmurs. Abdomen tender to palpation in right upper quadrant. Bowel sounds present. No abnormal findings on rectal exam, with stool negative for occult blood. Pulses equal and 2+ in all extremities. PERRLA. EOMs normal. Cranial nerves II - XII intact. No focal neurologic deficits. Ht 5' 7" (170 cm); weight 191 lb (87 kg); waist circumference 35 inches (89 cm); hip circumference 38 inches (96.5 cm).
