Further History (Celeste):

Your physical examination shows vital signs with T 36.8 C, P 73/min, RR 15/min, and BP 130/85 mm Hg. She is 155 cm tall and weighs 69 kg. She is alert and oriented. Her heart rate is regular with no murmurs. The lungs are clear to auscultation. Palpation of the abdomen reveals no masses or areas of tenderness, and active bowel sounds are present. The fundal height is consistent with 15 weeks. There is no lymphadenopathy. You note the presence of a 7 cm long transverse scar in the midline of the lower abdomen. The rectal examination reveals no masses, and the stool guaiac is negative. Pulses in the extremities are 2+ bilaterally. Motor strength is not decreased. Reflexes are normal. Cranial nerves II through XII are intact and there are no localizing neurologic signs. Funduscopic examination is unremarkable.

Your physical examination shows vital signs with T 38.4 C, P 85/min, RR 20/min, and BP 115/75 mm Hg. He is 170 cm tall and weighs 68 kg. He is alert and oriented but in mild respiratory distress. His heart rate is regular with no murmurs. The lungs have bibasilar crackles on auscultation. Palpation of the abdomen reveals no masses or areas of tenderness, and active bowel sounds are present. He has tender cervical lymphadenopathy. The rectal examination reveals no masses, and the stool guaiac is positive. Pulses in the extremities are 2+ bilaterally. Motor strength is not decreased. Reflexes are normal. Cranial nerves II through XII are intact and there are no localizing neurologic signs. Funduscopic examination is unremarkable.
