Introduction:

Your medical practice includes the San Giovanni family:

Family History:

The family owns a business, a restaurant, which serves lunch and dinner, seven days a week. Both Mike and Celeste work in the restaurant and are self employed. Each of them works 60 to 80 hours a week. Dionne is in day care.

History (Celeste):

Celeste is pregnant for the 2nd time. She visits you for a prenatal checkup. You order laboratory tests, including TORCH titers, HBsAg, and HIV. Your state requires a specific consent form for HIV testing. Celeste questions you about the need for HIV testing.

History (Mike):

A week later, Mike visits you because he has experienced a fever with cough and shortness of breath for the past 3 days. He has not been coughing up any sputum. He has had no major illnesses in the past, and describes himself as someone who never gets sick, because he cannot afford to miss a day working at the restaurant.
