This inflammation (mostly PMN's) is very subtle, but is associated with myocardial fiber necrosis. A fiber in the center shows a bluish appearance from dystrophic calcification. This patient has acute rejection associated with his transplanted heart. Endomyocardial biopsy is a very useful and necessary procedure to follow heart transplant patients to determine the degree of rejection.
