The umbilical cord normally has a spiral. However, excessive spiralling can obstruct the vessels, so-called cord torsion. Seen here is marked plethora in the obstructed segment of cord just before it enters the abdominal wall at the umbilicus. This resulted in intrauterine fetal demise, marked by the skin slippage seen here.
