This microscopic appearance shows nearly complete necrosis of epidermis and dermis, with only one set of sweat ducts remaining at the lower center. The significance of a "full thickness" burn injury in which all of the epidermis as well as the dermis including adnexal structures is necrotic is that, since the skin cannot regenerate from the adnexal structures, skin grafting is necessary. A "partial thickness" burn leaves adnexal structures from which the epidermis can regenerate.
