Case Introduction and Chief Complaint:

Your patient is a 54-year-old man who has had worsening joint aches and pains for the past 4 years. He complains of pain in both upper and lower extremities. He does not have significant back pain. He says the pain seems to get worse as the day progresses, but this is variable. His muscles only hurt when he engages in prolonged exercise, and that pain abates in a day or two. He says, "some days are better than others." He has tried celecoxib, but it doesn't help. He has had no major medical problems.
