This is a caseating granuloma caused by tuberculosis involving the adrenal gland. Tuberculosis used to be the most common cause for chronic adrenal insufficiency. Now, idiopathic (presumably autoimmune) Addison disease is much more often the cause for chronic adrenal insufficiency.
